    What you receive with the Personal Clinical Care Upon Hospital Discharge Package:

    On your loved one's hospital discharge day, our compassionate nurse stays from pickup through dinner time, creating a seamless transition home while teaching fall prevention, assessing the living environment, and ensuring proper wound care. 

    We handle all medication management—from pharmacy pickup to detailed explanations of each medication's purpose and side effects—while assessing nutritional needs, arranging food delivery, and providing crucial emotional support to both your parent and family. 

    The care continues by coordinating care with any health aides, ensuring your loved one receives consistent, knowledgeable care while giving your family peace of mind during this vulnerable transition.

    When is the best time to purchase:

    Upon the senior being diagnosed with a chronic or acute illness or

    When the senior is scheduled for a hospital-based surgery or

    When the senior is admitted to the hospital unexpectedly.

    Note that the earlier that we plan and coordinate services together, the more optimal care and support we can deliver to the senior and family

Many struggle to communicate symptoms or recall medical history, risking misdiagnosis and medication errors. Hospital environments often cause disorientation and complications like delirium.

    Without a family member or advocate to coordinate care, ask critical questions, and protect preferences, these vulnerable patients face increased risks of adverse outcomes and diminished dignity during their most vulnerable moments.

    What you receive with the Clinical Advocate Package:

    • Advocacy for the senior patient while in the hospital delivered by a nurse or social worker who acts as a bridge between patients and healthcare providers, navigates the complexities of the healthcare system, resolves issues, delivers personalized education, ensures the family is informed about their condition and ensures that the senior’s needs are met.

    • The Clinical Advocate also engages with the hospital staff to ensure that patients' rights are respected and protected, including the right to refuse treatment, access medical records, and make informed decisions. 

    • An especially important function of the Clinical Advocate is to offer emotional support to the senior patient and their families, recognizing that navigating a healthcare experience can be stressful and overwhelming.

    When is best time to purchase:

    Ideally, prior to age 65 and/or the senior(s) retirement and/or in advance of illness or medical procedures.

    Cost:

    $1,200 for any 4 hour period between 7 AM - 7 PM

    $1,800 for any 6 hour period between 7 AM - 7 PM

    $1,800 for any 4 hour period between 7 PM and 12 AM

    The Wellness Guardian package begins with a thorough initial assessment of your loved one's personalized needs—covering home safety, nutrition, exercise, mental health, and medication management—creating the foundation for their care plan. 

    Our caring professionals provide weekly in-home wellness visits complemented by several remote check-ins, supporting medication management while addressing emerging concerns before they become emergencies. 

    We support your parent's health through personalized engagement around their most pressing needs, coordinating preventive care appointments, and designing customized nutrition and exercise plans to promote holistic wellness. 

    Observations, interventions, and vital metrics are carefully documented, with monthly summary reports keeping you informed while maintaining your peace of mind.

    When is the best time to purchase:

    Preventative wellness services are encouraged at any age.

    Cost:

    $1,725 per month, with 3-month minimum commitment and payable at purchase.

Essential practical, legal, and medical information becomes urgently needed, yet may be difficult to access. Having this vital information readily available helps families provide optimal care for their senior loved one and navigate the complex challenges that emerge during these sensitive transitions.

    Without this proper planning, the families’ stress is exacerbated at the times where they need to focus and care for themselves and their families the most.   

    Peace of Mind Planner Package Value:

    • Establishes thoughtful foundations today for seamless support tomorrow.

    • During challenging transitions, thorough preparation allows families to focus on what matters most—caring for themselves and their loved ones—without the added burden of unnecessary stress.

    What you receive with Peace of Mind Planner Package:

    • A methodical and collaborative assessment of the existing and forecasted future state of organizational and medical readiness as it relates to you and your senior parents. 

    • Our analysis and preparation of a written report for you and your family, with actionable insights, to optimize the care for aging parents while best supporting your, and your families, existing responsibilities.   

    • A personalized strategy session to review the report and actionable insights with you and the appropriate family members who were included in the assessment.

    When is best time to purchase:

    Ideally before age 65 and prior to any acute and/or chronic diagnosis. 

    This is the first step in organizing vital information that takes time to complete.  The process can also become a significant emotional and logistical distraction, when everyone’s well-being needs to be prioritized.

    Cost:

    $1,150 one-time fee

  • As seniors age or are diagnosed with a medical condition, paying the bills on time becomes challenging. This is made even more complicated by the use of technology, online payment tools and medical portals, all of which feed the seniors' concern for scams and to pay the bills effectively and securely.

    Package Value:

    • Outsourcing ensures that bills are paid on time and that bill related issues are identified and resolved.

    What you receive with the Outsourced Bill Payment Package:

    • A dedicated account manager who will perform a comprehensive one-time assessment of bills that are to be paid, the amounts, processes and dates. 

    • Set up monthly account transfers and payees.

    • Payment of the bills to the appropriate payees per the amounts and dates specified during the setup process.

    • Quarterly reporting is provided to the family with monthly  information available upon request.

    When is the best time to purchase:

    The Outsourced Bill Pay Package may be purchased at any age.  The package alleviates concern for unforeseen financial-related issues that may unexpectedly arise due to major medical treatments or a traumatic injury or an accident requiring acute or sub-acute care or when the Senior Parent is moved into an Assisted Living Facility (ALF).

    Cost:

    $275 per month*, with minimum 3-month commitment to start

    * Includes up to 20 bills per month with an additional cost of $13.50 for each bill paid over 20 in a month.

    When is the best time to purchase:

    For the best experience, we recommend connecting with Silver Concierge about 3-4 months before turning 65, giving you ample time to understand your options before your Initial Enrollment Period begins. If you're already past 65, we're here to help you review your coverage before the Annual Enrollment Period (October 15-December 7), ensuring you can make beneficial changes for the coming year.

Individual Services

Individual services are sold in 10-hour packs, and may be used in any combination of the following services.   All services are delivered virtually through a video call or phone, unless noted otherwise.*

The cost for 10-hour packs is $2,850.

*On-site services are currently available in the New York City area and including Long Island, Westchester, and Western New Jersey.

    3. Safely remove dangerous emails including phishing, malware and spam

    4. Backup the data on devices and/or setup automatic backup services

    5. Setup new computers, phones, tablets, printers

    Minimum 1 hour from 10-hour pack

  • We will support the senior and family in the following services:

  • This service is designed for seniors who are focused on improving their health and wellness, or have specific needs as they may relate to diabetes or obesity (including those taking a GLP-1 medication).  This service will personalize the care for seniors aligned with their health, medical and fitness goals.   We will engage with the seniors to optimize their plans by integrating medical, physical mobility, food access, cultural sensitivities and medical guidance received from them and their physician.  

    For seniors with diabetes, the service includes learning how to use blood glucose meters, continuous glucose monitors (CGMs) and apps to interpret the data and understand the relationships between medications, food and exercise.

    For seniors taking GLP-1 medications, the service will support long-term weight loss through behavior modifications, managing through side effects and developing exercise regimens to minimize decreases in muscle mass associated with taking a GLP-1 medication. 

    This service is designed to educate and support the seniors in their day-to-day care and lifestyle.   While services are delivered by a person expert in nutrition, fitness, obesity and diabetes, this is not a medical service and will not receive reimbursement from a health plan.  All medical questions should be directed to the patient’s physician.

    Cost:

    Minimum 1 hour from 10-hour pack

  • Designed specifically for seniors who feel strained by emotional or physical stress, this service is focused on building stress resilience, while also improving sleep and joy.  Seniors who feel overwhelmed by loneliness or the passing of a spouse will find this service especially supportive.  This virtual service includes educating seniors on mindful movement postures and meditation techniques as well as practicing with them.

    While the services are delivered by an expert in Mindfulness and Emotional Support, this is not a medical service and will not receive reimbursement from a health plan.  All medical questions should be directed to the patient’s physician.

    Cost:

    Minimum 1 hour from 10-hour pack

  • A licensed clinician will provide a thorough health evaluation of your loved one, carefully reviewing their medical history, medications, and overall well being.  We'll prepare and personally review with you a clear, actionable report designed to enhance their quality of life and bring you peace of mind.

    We offer:

    • Professional health evaluation with personalized care recommendations

    • Optional screening of qualified caregivers for both daily assistance and respite care

    • Optional guidance in finding ideal living arrangements that truly meet your loved one's unique needs

    Cost:

    Minimum 3 hours from 10-hour pack

    Location and after-hours travel charges may apply

  • Manage the entire moving process, helping sort and pack belongings for relocations, downsizing, and life transitions while coordinating with bonded moving companies.

    Cost:

    Minimum 1 hour from 10-hour pack

  • Systematically organize vital senior information, including legal documents, insurance policies, medical records and other key information.

    For families who have completed the “Peace of Mind Planner” package, then this service is available in 1 hour increments using the 10-hour pack of hours.

    For families who have not completed the Peace of Mind Planner package, it is encouraged to do so for your time and cost efficiency.   Without the “Peace of Mind Planner” package, this service is available in 1 hour increments using the 10-pack of hours, after an initial 5 hour assessment and planning phase.

    Cost:

    Minimum 1 hour from 10-hour pack

  • Provide comprehensive review of medical bills and claim reimbursements, actively engaging with health plans to ensure accurate and timely claim processing.

    Services include:

    Your own Account Manager (AM) who understands the senior’s needs and insurance plans.   The AM will:

    • Proactively monitor the senior’s claim status to address any potential issues promptly

    • Communicate with the senior’s medical provider and insurance plan regarding any reimbursement discrepancies.

    • Manage reimbursement to address deductibles, co-pays, flexible spending accounts (FSAs), and coordination of benefits (COB) if there is primary and secondary insurance coverage.

    • Appealing any denied claims as appropriate. 

    Note that the AM will need to be added to your provider and insurance accounts to review claims due to HIPAA regulations.

    Cost:

    Minimum 2 hours from 10-hour pack per month.   Requires a three month minimum time commitment.
