Our bespoke Post-Discharge Concierge Service, delivered by a member of our nursing team, will provide the care outlined above and also will be able to address your loved one’s medical issues including wound care, drain management, chronic disease specific care and medication management.
Medicare Navigation and Plan Purchase Package | Not Applicable*
Our Medicare Navigation and Plan Purchase Package guides your family through the complex Medicare landscape with personalized support from a licensed New York State Accident and Health Broker, ideally beginning three months before reaching age 65. We provide a complimentary consultation to understand your unique healthcare needs, educate you on all available options, and ensure your preferred doctors and medications are covered while helping you avoid late enrollment penalties. Wellness Guardian | $2,500 for the initial assessment and personalized care plan. $250 per hour thereafter.
